The pathology and lab IDR record

CMS published 164,354 federal IDR disputes with pathology and laboratory line items for 2025. Groups won 81 percent of decided disputes, the median award was 5 times the qualifying payment amount across decided line items, and the median decision took 31 days. Quarterly volume grew roughly sixfold between late 2024 and late 2025, the fastest growth of any specialty.

A sixfold year

Pathology and laboratory filed 8,779 disputes in the third quarter of 2024. A year later the quarterly figure was 54,088, roughly six times as many, and the full 2025 count reached 164,354. Win rates held while volume exploded: 72 percent of decided disputes in late 2024, between 80 and 82 percent through every quarter of 2025. Growth like that is not payers getting worse; it is lab billers discovering that the No Surprises Act process works on small claims once they are batched.

The everyday test menu

The top codes are the ones every hospital lab runs all day. The complete blood count, 85025, appears in 93,474 disputes with a median QPA of $47.31 and a median decided award of $267.87. The comprehensive metabolic panel, 80053, appears in 74,243 with a $74.02 QPA and a $621.30 median award. Automated urinalysis (81003) was decided at 7.31 times its $25.56 benchmark, troponin (84484) at 4.56 times, rapid strep (87880) at 4.42 times, and the urine pregnancy test (81025) at 4.94 times. Arbitrators priced amylase (82150) at 7.53 times its QPA. None of these are exotic; the record is built on routine chemistry.

One payer is 38 percent of the docket

Blue Cross Blue Shield of Texas sat across the table in 62,662 of the 2025 disputes, 38 percent of the national pathology docket, and lost 82 percent of the decided ones at a median of 5.48 times the QPA. Cigna followed at 17,404 disputes, United Healthcare at 15,219, and Aetna at 13,994, where the median multiple was a lower 2.32. Geography matches the payer mix: Texas alone accounted for 97,844 disputes, about 60 percent of the national total.

What the dollars add up to

The median decided pathology line in 2025 carried a $49.09 QPA and a $265.25 award, and the year's decided awards on pathology and lab line items totaled roughly $171 million. For a lab group the math is volume math: hundreds of small lines, batched into disputes that each clear their fixed costs many times over.
